Procedure for application for freshmen year
BA studies
All applicants for Bachelor of Arts studies must submit the following documents to the appropriate department office no later than 31st May 2012 (the date on the postmark is what counts):
- Application form — to download as the pdf file: podanie (in Polish). It should be filled in using capital letters only.
- School-leaving examination certificate — original or copy certified by a school. The document is required to include an official information that it qualifies a candidate for studies in a country of issue of the school-leaving examination certificate (this information may be issued by educational authorities or Polish consulate in a country of issue of the school-leaving examination certificate or embassy or consulate of the country of issue in Poland).
The school-leaving examination certificate must moreover include an apostille or must be legalised (apostille or legalisation is a way to have legal documents mutually recognized by other countries; apostille is issued by educational authorities in a country of issue of the school-leaving examination certificate, legalisation is issued by a Polish consulate in a country of issue of the school-leaving examination certificate).
Candidates leaving school this year shall submit a certification stating that they have been allowed to sit their school-leaving examination and shall submit their school-leaving examination certificate no later than 16 July 2012 [non-negotiable deadline]. - Original of sworn translation into Polish language.
- Nostrification of the school-leaving examination certificate (which is issued in Poland by the Mazowieckie Kuratorium Oświaty www.kuratorium.waw.pl or a kuratorium competent with respect to the place of residence). - Medical certificate issued by a specialist in occupational medicine, according to the Minister of Heath and Social Welfare regulations of 11.08.2010 (medical examination requests are issued by the appropriate department office).
- Two up-to-date photographs (complying with regulations for identity card photographs), visible left semi-profile with the ear uncovered.
- Photocopy of identity card (twice enlarged).
- Photocopy of Karta Polaka (twice enlarged; if concerns), photocopy of karta pobytu (twice enlarged; if concerns).
- A document confirming knowledge of Polish language (one of the ones listed below): B1 Certificate (or higher), (more on: http://certyfikatpolski.pl) or Studium Języka Polskiego dla Cudzoziemców certificate or school-leaving certificate of a secondary school with Polish language as a language of instruction.
Before the enrolment procedure begins candidates must issue the examination fee. The exact amount will be given in the notification of permission to sit the examination.
The fee is not refundable.
The exact amount of fee for the enrolment procedure for the academic year 2012/2013 — will be given after being specified by the Ministry of Science and Higher Education.

- specialty: Symphony and Opera Conducting
- name of the composer and title of the performed work to be sent together with the application form
Department 2
- examination programme
Department 3
- list of completed material signed by tutor
- entrance examination programme
- scores of pieces to be performed during the examination and accompanied by the FCUM accompanist
Department 4
- singing examination programme (see: test material for the Department of Vocal Studies)
- scores of pieces to be performed during the examination and accompanied by the FCUM accompanist
Department 5
- specialties: Conducting Music Ensembles; Church Music
- scores of pieces to be conducted plus their piano reductions
- for the voice production examination one must submit the scores of the performed pieces
- the candidate must give the name of the instrument on which she or he will perform the examination and submit the scores of the performed pieces
Department 6
- audiometric test must be carried out at the FCUM before exams and its results should be submitted by the candidates Appointments should be made at the Chair of Musical Acoustics’s office. Phone: (48 22) 827 72 41 ext. 232. The test is free.
- scores or photocopies of piano scores of examination pieces if the candidate does not have his/her own accompanist (this applies to applicants who will be playing an instrument with piano accompaniment)
Department 7 in Białystok
- specialty: Instrumental Pedagogy
- list of completed material signed by tutor
- entrance examination programme
- scores (or their bound photocopies) of piano parts for the entrance examination (does not apply to pianists). Candidates who state in writing that they will sit the examination with their own accompanist are excused from this obligation
- specialty: Solo Singing
- entrance examination programme
- scores (or their bound photocopies) of piano parts for the entrance examination (does not apply to pianists). Candidates who state in writing that they will sit the examination with their own accompanist are excused from this obligation
- specialities Conducting Music Ensembles/School at Music/Church Music. For the exam testing conducting (manual) and vocal skills one should bring scores of the performed piece (2 copies).
MA studies
All applicants for Master of Arts studies must submit the following documents to the appropriate department office no later than 15th June 2012, personally or by post (the date on the postmark is what counts). Ballet Pedagogy or Choreography and Theory of Dance (extramural studies) submit the documents until 5th September 2012:
- Application form — to download as the pdf file: podanie (in Polish). It should be filled in using capital letters only.
- School-leaving examination certificate — original or copy issued by a school. The document is required to include an official information that it qualifies a candidate for studies in a country of issue of the school-leaving examination certificate (this information may be issued by educational authorities or Polish consulate in a country of issue of the school-leaving examination certificate or embassy or consulate of the country of issue in Poland).

- Diploma of at least Bachelor of Arts studies (BA Studies). In case of candidates finishing their BA Studies in the summer session of the academic year of 2011/2012 – a document certifying that they are admitted to sit the BA exam or that the exam is in progress. An original or a copy of the BA diploma should be submitted by 30th July 2012 [non-negotiable deadline]. For candidates for the Department of Sound Engineering only: Persons who will have their BA examination in the September session will be allowed to participate in the enrolment procedure conditionally, on the basis of the document certifying that they are allowed to sit the BA exam. A document from a candidate’s parent university certifying that she or he passed the BA examination will decide whether a candidate will be accepted or not and it should be submitted by 27th September 2012. An original or a copy of the diploma should be submitted by 31th October 2012.
The diploma must include an apostille or must be legalised (apostille or legalisation is a way to have legal documents mutually recognized by other countries; apostille is issued by educational authorities in a country of issue of the diploma, legalisation is issued by a Polish consulate in a country of issue of the diploma). - Original of sworn translation into Polish language of the diploma of Bachelor of Arts studies (BA studies).

- scores of the pieces conducted plus their piano transcriptions (two copies)
- for the voice production examination the candidates should submit the scores of the performed pieces
- specialty: Choreography and Theory of Dance
- In case a candidate has a BA diploma from a Major other than Dance it is necessary to certify their dance experience and submit documents certifying choreographic achievements within the area of dance (certificates of completion of courses, workshops or dance studies, certificates connected with dance, awards and competition diplomas)
- Attention: Before the exam a candidate is obliged to submit CDs with the piece the composition was prepared for to the Head of the Exam Committee
Department 7 in Białystok
- specialty: Instrumental Pedagogy
- list of completed material signed by tutor
- entrance examination programme
- scores (or their bound photocopies) of piano parts for the entrance examination (does not apply to pianists). Candidates who state in writing that they will sit the examination with their own accompanist are excused from this obligation
