Procedure for application for freshmen year

BA stud­ies

All appli­cants for Bach­e­lor of Arts stud­ies must sub­mit the fol­low­ing doc­u­ments to the appro­pri­ate depart­ment office no later than 31st May 2012 (the date on the post­mark is what counts):

  1. Appli­ca­tion form — to down­load as the pdf file: podanie (in Pol­ish). It should be filled in using cap­i­tal let­ters only.
  2. School-leaving exam­i­na­tion cer­tifi­cate — orig­i­nal or copy cer­ti­fied by a school. The doc­u­ment is required to include an offi­cial infor­ma­tion that it qual­i­fies a can­di­date for stud­ies in a coun­try of issue of the school-leaving exam­i­na­tion cer­tifi­cate (this infor­ma­tion may be issued by edu­ca­tional author­i­ties or Pol­ish con­sulate in a coun­try of issue of the school-leaving exam­i­na­tion cer­tifi­cate or embassy or con­sulate of the coun­try of issue in Poland).
    The school-leaving exam­i­na­tion cer­tifi­cate must more­over include an apos­tille or must be legalised (apos­tille or legal­i­sa­tion is a way to have legal doc­u­ments mutu­ally rec­og­nized by other coun­tries; apos­tille is issued by edu­ca­tional author­i­ties in a coun­try of issue of the school-leaving exam­i­na­tion cer­tifi­cate, legal­i­sa­tion is issued by a Pol­ish con­sulate in a coun­try of issue of the school-leaving exam­i­na­tion cer­tifi­cate).
    Can­di­dates leav­ing school this year shall sub­mit a cer­ti­fi­ca­tion stat­ing that they have been allowed to sit their school-leaving exam­i­na­tion and shall sub­mit their school-leaving exam­i­na­tion cer­tifi­cate no later than 16 July 2012 [non-negotiable deadline].
  3. Orig­i­nal of sworn trans­la­tion into Pol­ish language.
  4. Nos­tri­fi­ca­tion of the school-leaving exam­i­na­tion cer­tifi­cate (which is issued in Poland by the Mazowieckie Kura­to­rium Oświaty www.kuratorium.waw.pl or a kura­to­rium com­pe­tent with respect to the place of res­i­dence).   5. Med­ical cer­tifi­cate issued by a spe­cial­ist in occu­pa­tional med­i­cine, accord­ing to the Min­is­ter of Heath and Social Wel­fare reg­u­la­tions of 11.08.2010 (med­ical exam­i­na­tion requests are issued by the appro­pri­ate depart­ment office).
  6. Two up-to-date pho­tographs (com­ply­ing with reg­u­la­tions for iden­tity card pho­tographs), vis­i­ble left semi-profile with the ear uncov­ered.
  7. Pho­to­copy of iden­tity card (twice enlarged).
  8. Pho­to­copy of Karta Polaka (twice enlarged; if con­cerns), pho­to­copy of karta pobytu (twice enlarged; if concerns).
  9. A doc­u­ment con­firm­ing knowl­edge of Pol­ish lan­guage (one of the ones listed below): B1 Cer­tifi­cate (or higher), (more on: http://certyfikatpolski.pl) or Studium Języka Pol­skiego dla Cud­zoziem­ców cer­tifi­cate or school-leaving cer­tifi­cate of a sec­ondary school with Pol­ish lan­guage as a lan­guage of instruction.

Before the enrol­ment pro­ce­dure begins can­di­dates must issue the exam­i­na­tion fee. The exact amount will be given in the noti­fi­ca­tion of per­mis­sion to sit the exam­i­na­tion.
The fee is not refund­able.
The exact amount of fee for the enrol­ment pro­ce­dure for the aca­d­e­mic year 2012/2013 — will be given after being spec­i­fied by the Min­istry of Sci­ence and Higher Education.

MA stud­ies

All appli­cants for Mas­ter of Arts stud­ies must sub­mit the fol­low­ing doc­u­ments to the appro­pri­ate depart­ment office no later than 15th June 2012, per­son­ally or by post (the date on the post­mark is what counts). Bal­let Ped­a­gogy or Chore­og­ra­phy and The­ory of Dance (extra­mural stud­ies) sub­mit the doc­u­ments until 5th Sep­tem­ber 2012:

  5. Diploma of at least Bach­e­lor of Arts stud­ies (BA Stud­ies). In case of can­di­dates fin­ish­ing their BA Stud­ies in the sum­mer ses­sion of the aca­d­e­mic year of 2011/2012 – a doc­u­ment cer­ti­fy­ing that they are admit­ted to sit the BA exam or that the exam is in progress. An orig­i­nal or a copy of the BA diploma should be sub­mit­ted by 30th July 2012 [non-negotiable dead­line]. For can­di­dates for the Depart­ment of Sound Engi­neer­ing only: Per­sons who will have their BA exam­i­na­tion in the Sep­tem­ber ses­sion will be allowed to par­tic­i­pate in the enrol­ment pro­ce­dure con­di­tion­ally, on the basis of the doc­u­ment cer­ti­fy­ing that they are allowed to sit the BA exam. A doc­u­ment from a candidate’s par­ent uni­ver­sity cer­ti­fy­ing that she or he passed the BA exam­i­na­tion will decide whether a can­di­date will be accepted or not and it should be sub­mit­ted by 27th Sep­tem­ber 2012. An orig­i­nal or a copy of the diploma should be sub­mit­ted by 31th Octo­ber 2012.
    The diploma must include an apos­tille or must be legalised (apos­tille or legal­i­sa­tion is a way to have legal doc­u­ments mutu­ally rec­og­nized by other coun­tries; apos­tille is issued by edu­ca­tional author­i­ties in a coun­try of issue of the diploma, legal­i­sa­tion is issued by a Pol­ish con­sulate in a coun­try of issue of the diploma).
  6. Orig­i­nal of sworn trans­la­tion into Pol­ish lan­guage of the diploma of Bach­e­lor of Arts stud­ies (BA studies).
  7. Med­ical cer­tifi­cate issued by a spe­cial­ist in occu­pa­tional med­i­cine, accord­ing to the Min­is­ter of Heath and Social Wel­fare reg­u­la­tions of 11.08.2010 (med­ical exam­i­na­tion requests are issued by the appro­pri­ate depart­ment office).

    Atten­tion: can­di­dates apply­ing for Instru­men­tal Stud­ies, spe­cialty: Piano Play­ing with Eng­lish as the lan­guage of tuition are exempt from hav­ing to cer­tify their knowl­edge of Polish.
